Our clinic is located right in between the Legacy Parkway (Parrish Lane exit) and Interstate 15 at the Centerville Exit (319). 

From I-15:

Take exit 319. Go west from the freeway exit and take the first Right hand turn.  Then take your first Right hand turn onto 500 North, (by Roger's Marine).  We are the second brown brick building on your left hand side, with a big blue "Davis County Spinal Care" sign above the door.

From Legacy Parkway:

Take exit 7, Parrish Lane.  Go east from the exit and take your first left hand turn on to 1250 West (towards Roger's Marine).   Then take your first Right hand turn onto 500 North, (by Roger's Marine).  We are the second brown brick building on your left hand side, with a big blue "Davis County Spinal Care" sign above the door.
